The ANTHC Palliative Care Clinic is a specialty program that exemplifies the innovative ways that video visits can be leveraged to deliver patient care across Alaska. This presentation will provide an overview of how a multidisciplinary Palliative Care team has optimized video teleconferencing to provide end-of-life and supportive care to a difficult to reach patient population. Modalities that will be shared in this session include: scheduled virtual village clinic visits, direct to patient home visits, care conferencing with family members/providers/caregivers, discharge planning, and Project ECHO. The session will highlight the varying workflows, processes and equipment used for these modalities as well as how the Vidyo software and hardware infrastructure was rolled out across the state.
